The cornerstone of the Boston Health Care For The Homeless Program (BHCHP) is the trusting relationship built between patients and their doctors or clinicians. This relationship is fundamental to BHCHP’s approach to healthcare, ensuring that homeless individuals receive the comprehensive and respectful care they deserve. BHCHP operates as an integral part of Boston’s broader healthcare system, specifically addressing the unique health challenges faced by people experiencing homelessness. These challenges often arise from harsh living conditions, including exposure to extreme weather, experiences of trauma and violence, chronic illnesses, mental health conditions, and substance abuse issues.
Comprehensive Primary Care Services
BHCHP delivers vital primary care services at Massachusetts General Hospital (Mass General) each weekday through the Primary Care Urgent Access Center. Beyond immediate care, BHCHP actively coordinates ongoing treatment and discharge planning for homeless patients throughout the Mass General system. A key component of their services is the Barbara McInnis House, a unique facility providing recuperative and rehabilitative care. This house offers a safe haven for homeless individuals too sick or injured to recover on the streets or in shelters. The Barbara McInnis House effectively bridges a critical gap in the healthcare system, offering essential support to those who lack the stability of a home and family during recovery.
Reaching Patients Where They Are: The Street Team
Understanding that traditional clinic settings are not always accessible, BHCHP’s dedicated Street Team delivers direct medical care in diverse and unconventional locations. Operating out of Mass General, the Street Team professionals reach individuals living under bridges, in alleys, abandoned vehicles, parks, and at community meal programs. They also provide care within overnight drop-in centers, emergency departments, detoxification facilities, and nursing homes. This consistent presence on the streets establishes a reliable continuum of care, guiding individuals from initial street-level contact through intensive care, respite services, and ultimately, towards secure housing solutions.
Educating Future Physicians in Homeless Healthcare
BHCHP is committed to fostering the next generation of healthcare providers equipped to care for vulnerable populations. Through Mass General, BHCHP actively educates future physicians in the specialized skills and understanding required to treat homeless individuals. Primary care residents participate alongside BHCHP staff in community and street clinics as part of their ambulatory rotations. Furthermore, medical residents can choose a two or four-week elective rotation with BHCHP. This immersive experience allows them to work directly with BHCHP clinics and teams, gaining firsthand knowledge of the complex and specific healthcare needs of people experiencing homelessness.
