Participating in the CARE Card Program is straightforward. You can start earning rewards immediately by completing your new member health needs assessment or by undergoing any of the recommended health screenings or tests. It’s that simple to begin accumulating rewards that can be used for a variety of health and wellness products.

Here’s how members can earn rewards:

• For Children’s Health:

  • $20 for completing all six recommended well-child visits within the first 15 months of life.
  • $15 annually for attending well-child check-ups for children and teens aged 2 to 20 years.

• For Adult Wellness:

  • $20 annually for completing your annual wellness visit if you are 21 years or older.
  • $5 for getting your annual flu shot, helping you stay protected during flu season.

• For Women’s Health:

  • $20 for attending a postpartum check-up between 7 to 84 days after giving birth, ensuring maternal health post-delivery.
  • $10 for undergoing a cervical cancer screening, recommended for women aged 21 to 64, promoting preventative care.
  • $25 for attending a prenatal visit within the first 12 weeks of pregnancy, encouraging early prenatal care for healthy pregnancies.

• For Diabetes Management:

  • $10 annually for completing all three essential diabetes management tests: Hemoglobin A1c (HbA1c) testing, a dilated eye exam, and a kidney screening, supporting proactive diabetes care.

Activation Process:

To activate your CARE Card and start enjoying your rewards, you have two easy options:

1. Phone Activation: Call 1-888-682-2400 and follow the automated instructions provided.
2. Online Activation: Visit the website https://www.mybenefitscenter.com/ and follow the online activation prompts. You might need your health plan ID card handy during the activation process for verification.

Using Your Card at Participating Retailers:

Once your card is activated and loaded with rewards, you can use it at any retailer participating in the CARE Card program. The process is simple and discreet:

1. Shop for Eligible Items: Select from a wide range of eligible products available at participating stores. You don’t need a prescription and there’s no need to visit the pharmacy counter for eligible over-the-counter items.
2. Checkout and Payment: Proceed to the regular checkout lanes with your chosen items. Swipe your CARE Card just like a debit card to pay for your purchases. The card will cover eligible items up to your current balance.
3. Balance Management: Any unused balance on your CARE Card remains available for future purchases. As you earn more rewards by completing additional healthy activities, they will automatically be added to your card balance, ensuring your benefits are always accessible.

• Eye Care: Products to maintain eye health, such as contact lens solution, contact lens cases, eye drops, reading glasses, and sunglasses for eye protection.

• Children’s Over-The-Counter (OTC) Medicines: A range of medications specifically for children, including cough and cold remedies, allergy medicines, stomach relief, and children’s vitamins to support their health and well-being.

• Healthy Foods: Promoting nutrition with fresh produce, bottled water for hydration, meats, and dairy like milk and cream, encouraging healthy eating habits.

• Family Planning: Supplies for family planning and reproductive health, including pregnancy tests, condoms, and contraceptives.

• Women’s Care: Feminine hygiene and care products such as pads and tampons, panty liners, and anti-fungal creams for feminine health.

*Please note that CARE Card rewards cannot be used for purchases of alcohol, tobacco, or firearms. Rewards are valid for 12 months from the date they are earned or until member disenrollment. Rewards cannot be exchanged for cash.

The list of eligible CARE Card program rewards is subject to modifications. AmeriHealth Caritas Louisiana will provide advance notice of any changes.

Rewards are typically added to your card after your healthcare provider confirms completion of a qualifying healthy activity. Please be aware that not all members may be eligible to earn every reward listed.
