We bring expertise in geriatrics and palliative medicine directly to your home. As part of the UCSF teaching institution, our team is also involved in educating the next generation of healthcare providers. We focus on providing holistic support to patients and their families facing complex care needs. Our approach centers around understanding your personal goals and preferences for medical care, with a strong emphasis on effective symptom management and truly individualized care plans. Registered nurses and patient care coordinators are also integral members of our team, ensuring seamless and comprehensive support.
Our team possesses specialized training in managing common health challenges associated with aging. This includes conditions such as various forms of dementia (Alzheimer’s disease, vascular dementia, and others), arthritis, and heart problems, as well as addressing issues of frailty and falls to ensure patient safety and well-being within their home environment.

Referrals to the Care at Home program are welcomed and can be initiated from various sources, including UCSF hospitals and clinics, community-based physicians, home care agencies, family members, and adult protective services.
