  1. Complete and Submit Enrollment Documents: Return the completed Enrollment Agreement (and Disclosure Agreement, if applicable) along with your Sponsor’s Registration Agreement. Ensure all required information and signatures are provided to avoid processing delays.
  2. Tuition Payment: Submit your payment for the selected tuition plan via check, money order, or credit card. Include payment with your enrollment documents to finalize your application.

Important Enrollment Notes:

  • Complete Applications Required: Incomplete enrollment applications will be returned. Please ensure all sections are filled out accurately and completely.
  • Accuracy of Information: Any misrepresentation of facts within the Enrollment Agreement can result in immediate dismissal from the program. Honesty and accuracy are paramount.
  • Program Ownership: Program enrollment is owned by the party responsible for payment. If you personally fund your enrollment, you own it. If your employer pays, the company retains ownership of the enrollment.
  • Disclosure Agreement: The Disclosure Agreement is a legally binding document committing you to the full CPP tuition cost, regardless of program completion. Review this agreement carefully before signing.
  • Manual Delivery: Upon receipt of your Enrollment Agreement and fee, program manuals will be shipped via UPS within 10 business days, allowing you to begin your studies promptly.

Instructions for Completing Your Enrollment Agreement

Follow these steps carefully to ensure your Enrollment Agreement is correctly completed and processed efficiently:

  1. Application Form Completion: Fill out all sections of the Application for Enrollment form thoroughly. Do not leave any questions unanswered.

  2. Applicant Signature and Date: Sign your name and enter the date of signing in the designated space on the application.

  3. Sponsor Signature and NAO Membership: Obtain your sponsor’s signature on the Enrollment Agreement. Have your sponsor indicate their membership status with the National Academy of Opticianry (NAO) in the appropriate section.

  4. Sponsor Registration Agreement: Ensure your sponsor completes the Sponsor Registration Agreement in full.

  5. Payment Submission: Provide a check, money order, or credit card information for your selected tuition plan amount. If payment is made by a third party, clearly indicate your name and the program name (OCPP) on the payment instrument.

  6. Mailing Your Application: Mail the completed Enrollment Agreement, Sponsor Registration Agreement, and your payment to the National Academy of Opticianry at:
