What is the Community Care Program?
The Community Care Program is a financial assistance initiative designed to help individuals manage their healthcare financial responsibilities. It’s important to understand that Community Care is not solely based on income. We take a comprehensive approach, considering factors such as family size, outstanding balances, and an individual’s capacity to pay. At Fort HealthCare, we firmly believe that financial limitations should never be a barrier to receiving necessary medical services. Therefore, a discounted or sliding fee schedule is available, tailored to family size and income, to make healthcare more affordable.
Eligibility for the Community Care Program in Wisconsin
To determine eligibility for the Community Care Program, Fort HealthCare utilizes the Federal Poverty Guidelines, updated annually. These guidelines serve as a benchmark in assessing financial need. Notably, individuals and families with income levels up to 300 percent above the federal poverty guidelines may still qualify for Community Care. This demonstrates our commitment to providing a broad safety net for our community.
To begin the application process, we ask that you complete a Financial Questionnaire and provide the necessary supporting documentation. This information allows us to thoroughly evaluate your situation and determine your eligibility for Community Care and outline expected payment arrangements.
Applying for Community Care: What to Expect
Once we receive your completed application, you can expect to receive written notification of our Community Care decision within ten to fourteen days. This timeframe allows us to carefully review your application and make a fair determination regarding your eligibility.

Please be aware that Fort HealthCare reserves the right to withdraw Community Care approval if it is discovered that fraudulent information was provided on the application. Additionally, it’s important to note that Community Care approvals are specific to the services for which they are granted and do not automatically extend to future healthcare services provided by Fort HealthCare. A new application may be required for subsequent services.
