The State of Maryland is proactively transforming its healthcare system through the Total Cost of Care All-Payer Model agreement with the Federal Government. This initiative aims to improve patient care coordination across hospitals and other healthcare settings, ultimately leading to better health outcomes and controlled healthcare cost growth within Maryland. A cornerstone of this transformative model is the Maryland Primary Care Program (MDPCP).
MDPCP is a voluntary program designed for all eligible primary care providers in Maryland. It offers crucial funding and resources to facilitate the delivery of advanced primary care services throughout the state. By supporting primary care practices, the MDPCP empowers them to play a more significant role in preventative care, chronic disease management, and reducing avoidable hospitalizations.
The Maryland Department of Health (MDH) collaborated closely with the Centers for Medicare and Medicaid Services (CMS) to establish the MDPCP. The Program Management Office (PMO) within MDH’s Public Health Services, in partnership with the Center for Medicare and Medicaid Innovation (CMMI), is responsible for the implementation of this innovative healthcare transformation program.
Notably, CareFirst has joined the program as an aligned payer since January 2020, further strengthening the collaborative approach of MDPCP.
