How to Get Connected Through the Community Care Program

To begin your journey towards accessing the resources you need, simply submit one referral per household. Once you complete the form with information about your needs, dedicated program staff will connect you with a partner organization best suited to assist you. These partners are committed to reaching out to you, the client, within 1 to 3 business days to discuss your situation and explore available options. It’s important to remember that each program has its own eligibility criteria, and resource availability can vary depending on your location within Maine.

The Community Care Program is designed to connect you with ongoing support and resources. It is not intended for emergency situations. If you are experiencing a medical emergency, please contact your healthcare provider immediately, dial 911, or for a mental health crisis, call 988.

Your privacy is paramount. The information you share will only be used to connect you with local partners who can address your specific requests. Your data will not be shared with federal government agencies or immigration services. The Maine CDC is committed to data security and will delete your personal information within 90 days or when it is no longer needed to complete your referral, whichever timeframe is longer.
