Understanding the Patient Referral and Diagnostic Process

To ensure you receive the most appropriate care within our comprehensive memory care program, a structured diagnostic process is in place following a referral. Typically, this involves a minimum of three appointments designed to thoroughly assess your needs and develop a tailored care plan. We highly recommend that a family member or close friend, someone who knows you well, accompanies you to all appointments to provide valuable support and insight.

  • Initial Diagnostic Interview: Your first appointment will be a diagnostic interview involving you and your family member. During this session, we will carefully listen to your concerns and ask detailed questions about your overall physical and mental health. This conversation allows us to gain a deeper understanding of your personal goals and specific needs within our comprehensive memory care program. A cognitive screening test and a neurological exam may also be conducted at this stage. Depending on the initial assessment, further diagnostic testing, such as advanced brain imaging, comprehensive blood tests, and/or detailed neuropsychological testing, may be ordered to provide a complete picture.

  • Social Work Assessment for Family Caregivers: Recognizing the crucial role of family caregivers, the next step involves a dedicated appointment with a licensed clinical social worker for your family member or friend. Our commitment to whole-person care extends to supporting those who support you. This assessment focuses on understanding the needs and goals of family caregivers, ensuring they are also supported within our comprehensive memory care program.

  • Feedback and Personalized Care Plan Appointment: Finally, at the feedback visit, we will meticulously review all test results with you and your family member. We will provide a clear diagnosis based on our findings and present personalized recommendations tailored to your specific situation within our comprehensive memory care program. These recommendations may include medication management, vital brain health education, targeted cognitive-communication therapy, and/or supportive counseling services.

Once your condition is stable and a care plan is established, ongoing medical management will be seamlessly transitioned back to your primary care physician or referring physician, ensuring continuity of care. However, our specialized cognitive-communication therapy and social work counseling services remain readily available as needed to provide continued support within our comprehensive memory care program. We recognize that needs may evolve, and our team is always available to consult with you and your family members as your journey progresses. Convenient Self-Scheduling for Established Patients

For the convenience of our established patients, self-scheduling is now available for follow-up appointments. If you have previously been seen by a Comprehensive Memory Center provider and have an active MyUTHA Portal account, you can easily self-schedule your follow-up appointments. Appointments can be scheduled as early as 72 hours before your desired date and time, subject to availability. Information for New Patients Seeking Our Program

For those new to the Comprehensive Memory Center and interested in accessing our comprehensive memory care program, please note that all new patients require a referral from their healthcare provider. If you have any questions regarding the status of your referral or the referral process, please do not hesitate to contact the UT Health Austin Access Center at 1-833-UT-CARES (1-833-882-2737).
